Certificate Offering Circular & Application

Follow these simple steps to invest in the Georgia United Methodist Foundation's Certificate Program:

1. Download a copy of our Offering Circular, or contact the Foundation at info@gumf.org, 770-449-6726 or 877-220-5664.

2. Review the Offering Circular, which includes investment objectives, risks and other information.

3. Choose from four types of Applications to Purchase a Certificate. Click the link below to download the correct application form. 

• Custodian for a Minor Certificate Application
   
• Entity Certificate Application Fill-In
   
• Individuals Certificate Application Fill-In
   
• Trust Certificate Application
   

4. Complete the Application to Purchase a Certificate and mail it with your check made payable to the Georgia United Methodist Foundation. Our address is PO Box 922087, Peachtree Corners, GA 30010-2087.
